How we set it up

Send us your questions, the way you would write them on paper, and tell us which page or pages the form belongs on. We build it, put it on your site and send you a link to try it before anyone else sees it. Changing a question later is an email away.

A custom form is one page of questions, sent in one go. If you need payments, file uploads or questions that change depending on earlier answers, a dedicated form service is the better tool, and we can place it on your site for you.
